General Notice of Privacy Practice on Protected Health Information for Non-USA Citizens

1) All records that have confidential or medical information must be handled and discarded in a way that ensures the privacy and security of the records. 

2) Routine Consent for Release of Information: by enrolling in the Daedalos Institute, Ltd. Internet web site (daedalosinstitute.com) service and asking for a medical consultation you provide routine consent for the use of your personal medical information. This routine consent applies whether you enroll electronically, by telephone, by fax or by completing and signing an enrollment form. Your member information under this routine consent may be used for many purposes, including:

a) Delivery of health care.

b) Payment for consulting services to doctors and other providers

c) Measurement and improvement of our service

d) Clients' surveys

e) Investigation of complaints and appeals  

f) We may use non identifiable Information of your record for scientific purpose and statistics

3) Procedure for creating your Personal medical record Identification Number or PIN: To improve the confidentiality of your personal information, please submit  the completed  Personal Information FORM.   

We will use this information to create your Personal Medical Record and we will fax or mail your Personal Identification Number or PIN.  You will use this PIN when sending us Personal Information you wish to keep confidential. 

 4) Your Access to Medical Records: You or any person legally authorized by you may access your medical records by contacting us in writing with your or their signature confirmed by a public notary.

 5) We may release this information to medical consultants to enable them to deliver the services you requested. Our consultants are expected to implement confidentiality policies that address the disclosure of medical information, patient access to medical information, and the storage/protection of medical information. We will fax to you the practitioner's confidentiality processes and we will inform you of the name, the qualification and service charges of the consultant for your approval.
